Are formulas C5.39, C5.40 rearranged correctly? if vjh = (Pt^2+Ptfv)^0.5, the revised C5 C5.78, C5.79 seems like give more consistent arrangement.

Well spotted – they are not correct in the July 2017 version.
They were corrected in the errata published in March 2018 (Errata for Section C5 – The Seismic Assessment of Existing Buildings (eq-assess.org.nz)) with the corrected versions carried through to the “Yellow” version.
